INSURANCE CERTIFICATE
Applicants shall provide Mountain House Community Services District with a valid Certificate of Liability, including the endorsement page written through carriers acceptable to Mountain House Community Services District. Such certificate shall provide bodily injury and property damage liability protection at a minimum of $1,000,000 per occurrence. The applicant must be specified as the insured. If the use of alcohol is approved, you must include “Alcohol Coverage” in the policy (see the USE OF ALCOHOL section below). The certificate shall name the Mountain House Community Services District as an “additional insured” in conformance with the hold harmless agreement as outlined in the Application and must specify that the applicant’s insurance shall be primary to any insurance carried by the Mountain House Community Services District. The certificate shall be properly executed with the original signature of the authorizing agent of the insurance company. The insurance is due at least seven business days prior to rental use. Insurance can be obtained from your Homeowner’s Insurance company, or there are companies online that will sell Special Event Insurance. Please make sure that you list Mountain House CSD and the name of the rental location on the Certificate of Liability as the Certificate Holder. The address should be 251 E. Main Street. RESERVATION PENDING
I understand my reservation is pending until I provide the insurance no less than 7 (seven) days before my reservation. If I fail to provide the insurance, my reservation will be canceled, and cancelation fees may apply. I also understand that there may be multiple people who book this space. Whoever provides the insurance first will have an approved reservation if someone else makes a reservation for the same space and time. USE OF ALCOHOL
No person shall consume in any park any malt, spirituous or vinous liquor having an alcoholic content in excess of one (1) percent by volume, except as authorized by permit. (MH-4-1204) The following criteria have been established by which permits will be granted or denied: • Not to exceed six (6) hours of total usage • Restricted to beer and wine; distilled spirits are not permitted at any time • Must have an alcohol permit on file Alcohol sold by any means (selling tickets, cost in the admission price, donations, or charging a sponsor fee) also requires the user to obtain an Alcoholic Beverage Control (ABC) Liquor License. The ABC License must be on file with the Parks and Recreation Department before the renter is issued a Facility Use Permit with an Alcohol Permit. Security guards are required as a condition of an ABC license; 1 security guard per 150 guests. Alcohol served and/or sold at a function held in a District park, picnic or indoor facility requires a District Alcohol Permit and a Host Liquor Liability endorsement to the user’s General Liability insurance.
